RELEASE AND WAIVER OF LIABILITY
You must be at least 16 years old to be present on the construction site and under parent/guardian supervision. All volunteers must sign this waiver to be welcomed onto the construction site. Please ensure you are signing the correct waiver for your age group.
This Release and Waiver of Liability (the “Release”) is executed by (the “Volunteer”), in favor of York Habitat for Humanity, Habitat for Humanity International, Inc., and any other Habitat for Humanity affiliated organization, and their respective affiliates, directors, officers, trustees, employees, sponsors, donors, volunteers and agents (collectively, the “Released Parties”). The Volunteer desires to work as a volunteer for one or more of the Released Parties without compensation and engage in the activities related to being a volunteer. I understand that my activities may include but are not limited to the following: working at Habitat for Humanity offices and worksites; working in or for Habitat for Humanity ReStore operations; loading and unloading materials; traveling to and from work sites, towns, cities or countries; consuming food available or provided; living in housing provided for volunteers; assisting in disaster relief areas; constructing, repairing, and rehabilitating residential buildings; other construction- related activities; and other volunteer activities (“Activities”).
I, the Volunteer, hereby freely, voluntarily, and without duress execute this Release under the following terms:
Release and Waiver:
The volunteer does hereby release and forever discharge and hold harmless the Released Parties from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arises or may hereafter arise from Volunteer’s Activities. The Volunteer understands and acknowledges that this Release discharges York Habitat from any liability or claim that the Volunteer may have against York Habitat concerning any bodily injury, illness, death, or property damage that may result from Volunteer’s Activities. Volunteer also understands that York Habitat does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or insurance.
I, the Volunteer, further confirm that prior to engaging in the Activities, I watched the included safety video and reviewed the attached volunteer manual:

Medical Treatment:
The volunteer does nearby release and forever discharge York Habitat from any claim whatsoever which may arise from any first aid, treatment, or service rendered in connection with Volunteer’s Activities.
Assumption of Risk:
The volunteer understands that Activities may include those which may be hazardous to the Volunteer, including but not limited to construction, loading, unloading, uneven terrain, and transportation. Volunteer recognizes and understands that York Habitat’s various construction sites and ReStore may, in some situations, involve inherently dangerous activities. Volunteer nearby expressly and specifically assumes the risk of injury or harm in these Activities and releases York Habitat from all liability for injury, illness, death, or property damage resulting from the Activities.
Insurance:
The volunteer understands that York Habitat does not carry or maintain health, medical, or disability insurance coverage for any volunteer. The volunteer is responsible for carrying their own medical and disability insurance and ensuring that such insurances are in full effect on the date(s) of service.
Photographic Release:
The volunteer does hereby grant and convey unto York Habitat the right to publish any and all photographic images/video/audio recordings made by York Habitat during the Volunteer’s completion of Activities at York Habitat. Volunteer also grants unto Habitat the right, title, and interest in any and all royalties, proceeds, or other benefits derived from such media.
Other:
The volunteer expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the Commonwealth of Pennsylvania. Volunteer agrees that in the event that any clause or provision of this Release shall be held to be invalid by any court of the competent jurisdiction, the invalidity of such clause or provisions shall not otherwise affect the remaining provisions of this Release and shall continue to be enforceable.
Term: This Release and Waiver shall remain in full force and effect until it is revoked in writing.
